What is bone regeneration?
When there is insufficient residual bone to place an implant, Guided Bone Regeneration, medically called GBR (Guided Bone Regeneration), is used. GBR is an augmentation of bone and also gum volume, which is necessary to insert titanium posts (implants), which will be the roots of fixed prostheses.
Bone regeneration
Guided Bone Regeneration. Increased bone volumes.
The causes
Bone resorption following extraction or traumatic tooth fall.
The advantages
Functional, biological, aesthetic.
The cause of bone loss
The alveolar bone is an extremely spongy type of bone, which brings nourishment to the teeth and allows masticatory loads. It is functional for the presence of the tooth, and, if extracted, the bone resorbs quickly and three-dimensionally. The gingiva, which covers the bone, follows the resorption by adapting.
The bone continues to shrink, decreasing in quantity. As time passes without intervention, the reduction may affect other anatomical structures, such as the alveolar nerve and/or maxillary sinus.
Causes of bone loss can be:
traumatic tooth loss;
extractions;
periodontal diseases (pyorrhea);
peri-implantitis (infections around existing implants);
removable dentures worn for a long time;
trauma.
The intervention
The procedure involves the insertion of bone tissue grafts to restore biology, function, and aesthetics of the area. It can be done horizontally or vertically, depending on the need to increase thickness or height.
Using radiographs, CT scans, and impressions, one has the state of the art to construct the perfectly shaped, customized titanium grids that are surgically inserted into the space to be regenerated. Inside the grid, the person's own bone and synthetic composite is placed.
After about 8 months, thanks to the regenerative capacity of the human body (osteoblast cells), the bone reforms.
The grid is then removed and implants can be inserted.
The benefits
Restoring bone shape and volume allows implants to be placed.
The stages of the pathway
The dental examination
This is a thorough examination that searches for the causes of the lack of bone.
History (clinical data collection), radiographs, CT scans and impressions for proper assessment of the situation.
1
Post intervention
After about 8 months, thanks to the regenerative capacity of the human body (osteoblast cells), the bone reforms. The postoperative course is important to facilitate healing. It is recommended to take semi-liquid foods in the first 15 to 20 days after surgery, soft in the next 2 months. Avoid/limit coffee, alcohol and smoking, especially in the first 15 to 20 days after surgery. Avoid toothbrush use in the surgery area and use antibacterial mouthwashes for oral hygiene.

Bone regeneration surgery
The surgeon inserts custom titanium grids, perfectly shaped and customized into the space to be regenerated. Inside the grid, the person's own and synthetic bone compound is inserted. If lack of bone is not important, insertion of the grid can be avoided by placing the compound directly into the area to be treated, with a resorbable membrane to hold it in place.
2
Grid removal and implant insertion
When the bone has fully regenerated, the grid is removed and implants inserted.
